Nov. 1

F&P America, 2101 Corporate Dr., Troy — Observed employee drinks in an upright cooler sitting on all different levels of shelving. Observed the facility using raw fruit and vegetables without properly washing them. Observed bagels in display case without ingredient information available for customers. Bulk food display for customer self-service was not properly labeled. Observed cooking line equipment with residual food oil or build-up. Observed a hand sink that was not secure to the wall with caulking. Observed a build-up of dark residual on floor tile throughout food service.

Submarine House, 14 N. Market St., Troy — Observed ice machine with cracked lid. The surfaces of cutting boards were severely scratched and could not be effectively cleaned and sanitized. Observed dust, food, or residual build-up on food timers, vent hood, on top of nacho cheese dispensing machine, on fan in food service area, on knobs and back of slicer. Observed hand sink in bar being blocked with bucket of sanitizer. Critical violation: hand washing sink is being used for purposes other than hand washing.

UTC Aerospace, 101 Waco St., Troy — Observed packaged foods in display cooler not labeled with ingredients. The surfaces of cutting boards were severely scratched and could not be effectively cleaned and sanitized. Critical violation: observed food slicer with residual build-up on back areas of slicer parts and blade. Observed accumulation of soil residue on nonfood contact surfaces of the cooler or freezer door gaskets. Single use articles are not properly stored, observed pizza boxes sitting on floor. Observed residual build-up on floor of stock room.

Nov. 3

Mr. Deals Supermarket, 689 S. Miami St., West Milton — Critical violation: observed raw chicken and raw meat next to and above containers of ready to eat foods in the case. Critical violation: TCS foods were not being held at the proper temperature. Observed meatballs and green beans in hot holding at 115F, instead of the required 135F or above. Critical violation: ready-to-eat, TCS foods that had been date marked were not properly discarded when required. Observed ham salad with a date of Oct. 22, which exceeds seven days.

Rich Oil, 1 W. Broadway St., Covington — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Critical violation: observed residue inside of both ice chutes on beverage dispenser. Observed dust and debris gathering in metal grating inside of cold holding machine. Observed food residue and residue stains on the top of the microwave and above the microwave door gasket. Critical violation: hand washing sink was not easily accessible. Observed soda syrup spilling underneath rack of soda boxes in the back storage area.
